Finasteride and blood donation: deferral is about birth defects

Finasteride and dutasteride can cause birth defects involving the external genitalia of a male fetus if a pregnant person is exposed. That is why crushed tablets are a handling hazard and why labels warn about semen exposure in some settings. Blood centers apply a related logic: if a unit from a recent 5-ARI user were transfused to a person who is pregnant, residual drug is treated as an unacceptable risk. American Red Cross and other AABB-aligned educational materials have long listed finasteride with a 1-month wait after the last dose and dutasteride (Avodart) with a 6-month wait. Always answer the donor questionnaire honestly. A hair-loss tablet counts. A prostate tablet counts. A 'I only take it three times a week' pattern still counts.

Rules can differ by country and by collection organization. If you donate platelets or plasma, ask that center — do not assume whole-blood timing. Stopping finasteride only to donate, then restarting, will also restart the hair-loss clock for many people; that trade is yours and your clinician's, not a stranger's at the snack table. Do not donate if you are donating to 'flush' the drug; that is not how pharmacokinetics or blood safety works. List every medicine, including topical compounded finasteride if you use it — staff would rather hear a maybe than discover a label in your bag after you have already given.
