A dramatic shed after COVID is common and usually a delayed telogen effluvium, not the virus permanently attacking every follicle. Density often returns over months once you have recovered.
High fever and significant illness — including COVID-19 — are classic TE triggers. Follicles synchronise into the resting phase and shed together about two to three months later. The AAD has discussed shedding after illness in general terms: handfuls in the shower, a thinner ponytail, usually diffuse rather than a neat Norwood map. It is not proof of a new scarring disease by itself. Hospitalisation, weight loss, and new medicines can stack extra triggers. How long: many people see the daily storm ease over subsequent months, with cosmetic density improving across about six to twelve months as new hairs grow in. The calendar is not a guarantee. If a year later you have a clearly receding hairline or a widening part, illness may have unmasked androgenetic alopecia that still needs its own plan. Check ferritin and thyroid if recovery is slow and your clinician agrees those tests fit. Minoxidil is sometimes used to support regrowth; it is a drug decision, not a COVID souvenir from social media.
Vaccination can also be followed by TE in some reports, usually in the same delayed pattern; that does not mean you should skip indicated vaccines to 'save hair.' Should I start finasteride the week I test positive?
No. Acute infection is not the moment to begin a DHT blocker without a clinician. Wait until you are stable, confirm the hair diagnosis, and discuss pregnancy and sexual side-effect counselling if that drug is even appropriate.
