Does ketoconazole help a receding hairline?

In the United States, ketoconazole 1% shampoo (Nizoral A-D and generics) is an over-the-counter Drug Facts product for dandruff. Two-percent shampoo is typically prescription and is used for seborrheic dermatitis — Malassezia-associated scale and itch. The American Academy of Dermatology treats ketoconazole shampoo as antifungal/anti-inflammatory scalp care, not as a licensed hair-regrowth drug. U.S. minoxidil foam and solution labels are the OTC standard for androgenetic thinning. This page will not invent a millilitre of leave-on time beyond the carton.

People apply ketoconazole at the temples because small randomized work — including a 1998 Dermatology comparison of 2% ketoconazole shampoo by Piérard-Franchimont and colleagues — reported hair-density signals in androgenetic alopecia. Those are adjunct data, not a Rogaine substitute and not an FDA hair-loss indication. Proposed mechanisms include reduced scalp inflammation and a weak anti-androgen effect in vitro; that is a hypothesis, not a DHT-blood-test protocol. Contact dermatitis, dryness, and hair-texture change can happen. Rinse as directed; leaving shampoo on “all night for the hairline” is not a labeled schedule.

Do not confuse shampoo with oral ketoconazole tablets (historically Nizoral tablets). The FDA restricted oral ketoconazole and boxed hepatotoxicity and drug-interaction warnings — that is not a hairline hack and not something to borrow from a leftover bottle. Frontal fibrosing alopecia and traction need a diagnosis, not a stronger wash. If the hairline is shiny and scarred, or eyebrows are thinning, see a dermatologist. Pregnancy: ask the obstetric clinician before any medicated shampoo routine.
