Pick the antifungal your scalp actually needs. Do not treat a 2% shampoo as if it were finasteride in a bottle.
Ketoconazole shampoo is an imidazole antifungal. In the U.S., 1% is widely sold over the counter and 2% is often prescription. It is used for seborrheic dermatitis and dandruff — itchy, greasy scale that can make shedding look worse and make minoxidil sting. Some small hair studies have looked at ketoconazole as an adjunct in pattern loss; that is not the same as a labeled AGA monotherapy. Ciclopirox (often ciclopirox olamine) shampoo is another antifungal, typically prescription in many markets, with a different mechanism (including metal-ion chelation) and a solid seborrheic-dermatitis job. If your problem is yeast-driven scale, either drug class can be appropriate under a clinician. If your problem is a receding temple with a calm scalp, swapping shampoos will not rebuild the hairline.
Practical differences are access, contact time, and what else is on the label. Follow the wash-on minutes the product states; a 30-second rinse is how people decide 'it failed.' Alternate days with a gentle cleanser if everyday use dries you out. Do not stack two prescription antifungals plus a tar plus a steroid plus daily minoxidil on raw skin without a plan — you will create a new dermatitis and then blame the last bottle. Children, pregnancy, and broken skin change which agent and which strength are reasonable; that is a clinician call. Ketoconazole tablets (oral) are a different, more systemic conversation than the shampoo and are not a DIY hair protocol.
