A severe form of alopecia areata in which the entire scalp loses its hair. Like alopecia areata it is autoimmune; newer oral JAK inhibitors have given some patients meaningful regrowth. It requires a dermatologist and is unrelated to pattern baldness.

- Broken hairs or rapid loss
- Loss with body-wide signs (weight loss, fatigue, cycle changes, acne, extra hair)
- Loss right after a new medication
- Any hair loss in a child
