Saw palmetto (Serenoa repens) is sold for prostate symptoms and, by extension, as a ‘natural finasteride.’ Extracts may have some 5-alpha-reductase activity in vitro. Human hair trials are few, often tiny, sometimes combined with other ingredients, and not a substitute for the large finasteride programmes that led to FDA approval. People who cannot take finasteride sometimes try saw palmetto; they should know they are choosing a lower-evidence path, not a gentler twin of the drug.
Side effects can include stomach upset and, theoretically, hormonal effects; it is not automatically safe in pregnancy or before surgery (bleeding discussions exist in herbal literature). Supplement labels are poorly standardised — dose and extract type differ. Do not stack it with finasteride assuming double DHT blockade is smarter; tell your clinician everything you take.
