A cold cap is oncology supportive care. Success is 'more hair kept,' not a promise of a perfect hairline on infusion day.
Scalp cooling constricts blood vessels in the scalp so less chemotherapy reaches follicles, and it may reduce follicular metabolism. Devices such as DigniCap and Paxman systems have been cleared by the U.S. FDA to reduce chemo-related hair loss in appropriate patients. That clearance is not a guarantee you will keep a full head of hair. Benefit is better documented for certain solid-tumor regimens than for others. Many hematologic cancers and some drug combinations are poor or contraindicated fits because of concerns about hiding scalp metastases or because the protocol simply does not support cooling. Your oncologist, not a beauty blog, decides eligibility.
The session is long and cold: fitting, cooling before the drip, remaining cold during and after. Some people stop because of headache, cold intolerance, or cost and chair time. Insurance coverage varies. Manual dry-ice caps exist alongside machine systems; technique and temperature control differ, and DIY social-media versions are how people get frost injury. Cooling does not treat androgenetic alopecia you already had, and it does not replace antiemetics or growth-factor decisions. After the course, remaining hair can still shed on a delay, and texture can change. Wigs remain a valid plan A or plan B.
This is not oncology advice. Ask the infusion center weeks ahead — machines are booked. Do not start minoxidil or oils on a cooling day unless the team said so. Fever, a painful scalp plaque, or sudden neurologic symptoms are medical, not a reason to tighten the cap. When treatment ends, a dermatologist can help with the new hair and with any pattern loss that was waiting underneath.
