If you need a hairline this Friday, a system wins. If you want living hair in a scar-aware, donor-honest plan, surgery is a different conversation.

Medical wigs, toppers, and full lace systems are devices. They do not change follicles. That is a feature when the diagnosis is chemotherapy, alopecia areata, or a scalp that is not a transplant candidate. Modern systems can look excellent at conversational distance when the base, color, and hairline are built for you. Costs repeat: adhesive or clips, servicing, heat, gym and swim rules, and eventual replacement. Insurance or charity help exists for some cancer-related loss; it is rarely a blank check for cosmetic pattern loss. A transplant, by contrast, moves DHT-resistant follicles. Those hairs can grow for years, but the native untreated hair can still thin, and the donor is finite.

Surgery wants a diagnosis of stable-enough androgenetic loss (or a reconstructed scar), enough donor, and months of patience: shed, then growth, then a 9–12 month verdict. It will not rebuild a Norwood 7 into a teenage density. It will not stop alopecia areata if the immune attack is active. It is a poor first move when you have not tried or cannot use medical therapy and the hairline is still racing. A wig does not require a donor. It does require maintenance honesty. Some people wear a system while grafts grow, or wear a system on the crown and transplant only the hairline. That is strategy, not failure.

Scalp infection, adhesive allergy, and traction from a too-tight wig are real. Transplant infection and overharvest are real. Do not let a salesperson talk you out of a dermatology diagnosis. Photograph your goals in daylight. If the budget is one shot, decide whether that shot is living grafts or a year of high-quality coverage — they are not interchangeable SKUs.
