A circular bald spot can be either. The wrong fork wastes a year of minoxidil for a pulling disorder, or shames an autoimmune patient for “nervous habits.”
The American Academy of Dermatology describes alopecia areata as non-scarring patches, exclamation-mark hairs, and sometimes nail pitting; severe forms include totalis and universalis. Trichotillomania (hair-pulling disorder) is classified among body-focused repetitive behaviours; hairs are broken at different lengths, and there may be a bizarre pattern the patient can map to a hand. Do not accuse a child of pulling without a gentle exam. Do not start finasteride for circular patches. Minoxidil is sometimes used as an adjunct in areata under a clinician; it is not a pulling cure. Cognitive-behavioural approaches and habit-reversal are the pulling literature’s core, not a 5-ARI.

Can they coexist?
