Ketoconazole shampoo can help hair look and feel better when dandruff or seborrheic dermatitis is inflaming the scalp. It is not a proven replacement for FDA-approved pattern-loss drugs.

Ketoconazole is an antifungal. In shampoos it reduces Malassezia yeast and the redness, grease, and scale of seborrheic dermatitis — a condition the American Academy of Dermatology discusses as a common, treatable scalp problem. When the scalp is inflamed, hairs can shed in a telogen-effluvium pattern or simply look thinner because they are coated and broken. Calming that disease is real hair help. Interest in ketoconazole for androgenetic alopecia comes from anti-inflammatory effects and laboratory hints of mild anti-androgen activity at the follicle. The human hair-count literature is small. Marketing that ranks Nizoral next to prescription DHT blockers is advertising, not a label.

In the US, 1% ketoconazole is often sold over the counter and 2% is commonly prescription; other countries reverse or restrict strengths. A typical routine is two or three washes a week, lather on the scalp, leave a few minutes, then rinse — not a ten-minute 'treatment mask' every morning unless a clinician said so. Daily harsh use can dry shafts even if the scalp feels cleaner. Brand name matters less than the ingredient and how you use it. People who are pregnant, treating a child, or using other medicated topicals should ask a pharmacist or clinician before stacking antifungals, tar, and steroids.

If your hairline is receding in a male-pattern or the part is widening in a female-pattern, ketoconazole can sit beside proven therapy. It should not be the only plan. Round bald patches, thick yellow scale that bleeds, pain, or pus are not a shampoo DIY project. This is not medical advice.
