Seeing more hair in the shower a month after FUE or FUT frightens people. The follicles that were transplanted often go quiet before they grow. Native hairs next to incisions can join the shed.
Operative trauma, inflammation, and a sudden change in local blood flow can push follicles into telogen. Surgeons call this shock loss. Timing is often a few weeks to a couple of months after the procedure — in the same family as other telogen sheds the American Academy of Dermatology describes, even though the trigger here is surgery. Grafts typically shed their shafts and then cycle; counting “lost grafts” in week six from hairs in the drain is how people panic incorrectly. Photographs at 6–12 months are the usual growth clock for transplanted units, not week two.
Risk is higher when the recipient still has a lot of miniaturized native hair (the surgeon is working in a thinning zone, not a bald patch). Aggressive recipient slitting, infection, and poor donor handling are surgical quality issues. Minoxidil is sometimes paused and restarted; that pause can add a shed. This is not medical advice. Call the operating clinic for pus, expanding redness, or a fever — those are not “normal shock.”
Permanent loss of native hair can occur if blood supply is compromised or if miniaturized hairs were going to die anyway. A second procedure is not an emergency because of ordinary shock. Will native hair come back after shock loss?

Can minoxidil prevent shock loss?
Some surgeons use it around surgery; evidence is mixed and protocols differ. Follow the written plan, not a stacked extra foam coat.
Is shock loss the same as a botched hairline?
No. A badly designed hairline is an aesthetic plan. Shock loss is a shedding phase. Both can coexist; they are not synonyms.
