Telogen effluvium vs androgenetic hair loss

💡 まず結論

A shower full of hair is not automatically pattern baldness. Telogen effluvium is a timed, usually diffuse shed. Androgenetic alopecia is gradual miniaturisation in a pattern. They can happen together.

Telogen effluvium (TE) happens when a large group of follicles shift into the resting phase after a trigger — high fever, surgery, crash diet, a new medicine, postpartum hormone change, or major physiologic stress. Shed often arrives about two to three months later and is typically all-over rather than a neat receding temple. A gentle pull test may loosen many telogen hairs; that is a clinic clue, not a home game. NIH-hosted clinical reviews describe TE as usually self-limited once the trigger is gone. Androgenetic alopecia (AGA) is genetic sensitivity to DHT: hair shafts get thinner over years at the hairline, crown, or widening part. You do not need a dramatic illness for AGA to progress.

Overlap is common. Illness can unmask a hairline you were going to notice later. TE can sit on top of Ludwig thinning so the part looks suddenly worse. Clues that lean AGA: miniaturised hairs on dermoscopy, a family pattern, slow change over years, temples or vertex mapping to Norwood or Ludwig stages. Clues that lean TE: a dated trigger, sudden daily handfuls, relatively preserved hairline shape, and improvement over six to twelve months. Scarring, pustules, and coin-shaped bald patches are neither typical TE nor typical AGA — those need prompt dermatology.

Do not start finasteride because a video said every shed is DHT. Do not ignore a receding hairline because someone said 'it is only stress.' Blood tests (iron/ferritin, thyroid) are for unexplained diffuse shed, not a substitute for looking at the pattern. This is not medical advice. AAD patient pages on shedding and pattern loss are a safer map than stacked supplements. Treatment, if any, follows the diagnosis: TE needs time and trigger repair; AGA needs therapies with outcome data, chosen with a clinician.

よくある質問

Can I have telogen effluvium and pattern loss at the same time?

Yes. A fever or postpartum shed can empty the drain while miniaturisation quietly continues at the crown or temples. That is why photos and a scalp exam beat a single viral explanation.

How long does telogen effluvium last compared with androgenetic loss?

Classic TE often eases over months after the trigger ends, though the calendar is not a stopwatch. AGA does not have a six-month finish line; it tends to progress without treatment. If 'TE' is still worsening in a clear pattern a year later, revisit the label.

Does a pull test diagnose androgenetic alopecia?

No. A pull test looks at how many hairs are ready to shed. Pattern loss is judged by distribution and miniaturisation. Do not yank your own scalp raw trying to self-diagnose.
