A slowly receding frontal hairline with shiny, scarred skin and often eyebrow loss is a different disease from androgenetic alopecia. FFA can permanently destroy follicles if untreated.

Frontal fibrosing alopecia is a primary lymphocytic scarring alopecia. The band of hair along the front and sides recedes; the skin can look pale or atrophic, and lonely hairs or redness at the hairline are clues. Eyebrows commonly thin. It is seen most often in postmenopausal women but is not exclusive to them. Many dermatologists consider it a patterned form of lichen planopilaris. Possible associations (leave-on facial products, sunscreen chemicals) are researched and not settled; do not panic-throw every product without a clinician’s plan. Minoxidil and anti-androgens that help pattern loss do not, by themselves, stop a scarring process.

Diagnosis may need trichoscopy and sometimes a scalp biopsy. Treatment aims to quiet inflammation and slow further loss: topical or injected corticosteroids, calcineurin inhibitors, tetracycline-class antibiotics as anti-inflammatories, hydroxychloroquine, and 5-alpha-reductase inhibitors in selected patients are among options dermatologists use — none is a simple OTC protocol, and JAK inhibitors for alopecia areata are a different indication. Hair transplant into active FFA can fail or look odd; surgeons usually want disease control first. Is frontal fibrosing alopecia the same as a mature hairline?

No. A mature hairline is a non-scarring, usually early-adult change in men. FFA is inflammatory scarring that can also take eyebrows and leave a band of bare, shiny skin.

Can FFA hair grow back?

Once a follicle is scarred, that unit does not reliably return. Early treatment tries to save remaining hairs. Expectations should be about halting spread, not restoring a teenage hairline.
