Androgenetic alopecia (male or female pattern) is follicle miniaturization in a genetically primed distribution: temples and crown in many men, a widening part in many women. It is slow, usually not painful, and the scalp skin looks normal. First-line medicines are topical minoxidil and, for appropriate men, a 5-ARI. Alopecia areata is autoimmune: T cells target the follicle, classically as round smooth patches, sometimes a band (ophiasis), sometimes the whole scalp (totalis) or body (universalis). Nails can pit. AAD pages and recent approvals matter here: oral JAK inhibitors such as baricitinib, ritlecitinib, and deuruxolitinib are options for severe areata under specialists, not starter kits for a receding temple.

Overlap exists and fools people. Diffuse areata can look like a bad telogen or a sudden FPHL. Pattern loss can coexist with a true areata patch. Pull test, dermoscopy (exclamation-mark hairs, yellow dots versus miniaturized hairs), and sometimes a biopsy separate them. Steroid injections can help localized areata; they do not rebuild a Norwood 5. Finasteride does not treat areata. Minoxidil is sometimes used as an adjunct in areata but is not the immune reset. Family history of pattern loss does not rule out areata, and a stressful month does not prove the patches are 'just stress TE' if the patches are bald and smooth.

This is not a diagnosis. Sudden coin-shaped bald spots, eyelash loss, or a rapidly emptying scalp belong with a dermatologist this week, not a 6-month minoxidil experiment copied from a male forum. Pattern thinning over years belongs on the pattern pathway. If a clinic offers the same PRP package for every bald spot, ask which disease they think you have. Pregnancy, children, and JAK-inhibitor infection risks are specialist conversations. Photograph both the hairline and any patches in the same light.
